INSTRUCTIONS

 

  1. PLEASE PRINT AND COMPLETE THE OFF DUTY REQUEST FORM BY CLICKING THE LINK BELOW.
  2. RETURN THE FORM WITHIN 14 DAYS OF YOUR EVENT TO THE SEMINOLE POLICE DEPARTMENT OR FAX TO 432-758-3876.
  3. THE FORM WILL BE EVALUATED AND EITHER APPROVED OR DENIED BY THE CHIEF OF POLICE OR HIS DESIGNEE BASED ON THE PROVIDED INFORMATION.
  4. UPON APPROVAL, YOU WILL BE NOTIFIED OF APPROVAL AND YOUR EVENT WILL BE SCHEDULED.
  5. IF DENIED, YOU WILL ALSO BE NOTIFIED AND GIVEN THE REASON. IF CHANGES NEED TO BE MADE IN ORDER TO GAIN APPROVAL, YOU WILL BE GIVEN THE OPPORTUNITY TO MAKE THOSE CHANGES. IF YOU DO NOT WISH TO MAKE THE REQUIRED CHANGES, SECURITY WILL NOT BE PROVIDED.
